SCHEDULE AN APPOINTMENT

All patients at Woven Health Clinic are seen by appointment only. Appointments for new patients are $25.00 and must be paid at the time the appointment is scheduled. Appointments for returning patients are $20.00 and are paid the day of the visit. The Clinic accepts cash, credit and debit cards.

To schedule an appointment, please call (972) 484-4444. Our phone is answered Monday through Thursday, 8:30 am to 12:30 pm and 1:30 to 5:00 pm, and Friday, 8:30 am to 1:00 pm. The Clinic sees patients Monday through Thursday from 9:00 am to 5:00 pm and Friday from 9:00 am to 1:00 pm. To schedule an appointment, please call (972) 484-8444 during business hours. The Clinic sees patients by appointment only. For returning patients, some same-day appointments are available on a first-call, first-serve basis.

New patients, please bring the following to your appointment:

  • All medications that you are currently taking
  • The name, phone number and address of your preferred pharmacy
  • The name and phone number of your previous physician
  • Medical records from your previous physician — please have these faxed, emailed or mailed prior to your visit
  • Medical records from any emergency room visits within the last 12 months
  • A form of identification with your picture (driver’s license, passport, matriculate, student ID)
  • Proof of income
  • Completed new patient forms (click here to download)

Returning patients, please bring the following to your appointment:

  • All medications that you are currently taking
  • A form of identification with your picture (driver’s license, passport, matriculate, student ID)
  • Medical records from any emergency room visits since your last appointment
  • Completed returning patient forms (click here to download)

We strive to be flexible and accessible for our patients, and recognize that sometimes patients will need to reschedule or cancel appointments. In order to best serve all our patients, scheduled appointments must be cancelled or rescheduled no less than 48 hours prior to the appointment during business hours. New patients who do not cancel or reschedule 48 hours prior to their appointment during business hours will be charged a $25.00 no-show fee. Returning patients who do not cancel or reschedule 48 hours during business hours prior to their appointment will be charged a $20.00 no-show fee at their next appointment.

First-time visit: $25

Regular appointment: $20

Well-women visit: $50

Appointment no-show or canelation fee: $20
